Please use this page to prepare the room before I arrive. The operation is a laparoscopic repair of a left ureteroenteric (uretero-ileal) stricture in a patient with an ileal conduit and a left nephrostomy. There is no robot.

Table. An operating table that tilts head-down and rotates side to side. Anti-slip mattress (gel or foam) directly under the patient: the patient will be head-down with the table rotated.Mesa. Supine. Legs straight and together. Table flat, no flex: the flank position is not used.Decúbito supino (dorsal). Piernas extendidas y juntas. Mesa sin flexión: no se usa la posición de flanco.
Roll under the left side. A rolled sheet or gel roll under the left flank and left buttock, to raise the left side. Keep the nephrostomy tube off the roll: not kinked, not compressed.Rollo bajo el lado izquierdo. Rollo de sábana o de gel bajo el flanco y el glúteo izquierdos, para elevar el lado izquierdo. La sonda de nefrostomía no debe quedar bajo el rollo: sin acodarse y sin compresión.
Arms. Both arms tucked at the sides, palms toward the thighs, wrapped in the sheet. Pad the elbows (ulnar nerve) and the hands. IV and arterial lines stay reachable for anesthesia. The surgeon and the camera holder stand on the right and the assistant on the left, so neither arm can go out on an arm board.Brazos. Straps and padding. A strap across the chest over padding, and one across the thighs. No shoulder braces against the neck. Pad the heels and every bony point. Eyes closed and protected.Cintas y acojinamiento. Cinta de sujeción sobre el tórax con acojinamiento, y otra sobre los muslos. Sin hombreras contra el cuello. Tilt test. Before draping, test full Trendelenburg and right-side-down rotation with anesthesia watching. The patient must not slide.Prueba de inclinación. Stomach. Orogastric tube after intubation, to empty the stomach before the trocars go in.Estómago. Sonda orogástrica después de la intubación, para vaciar el estómago antes de colocar los trocares.
Stoma. After induction, remove the urostomy bag and prep the stoma into the sterile field. The flexible cystoscope, or a catheter, goes in through the stoma.Estoma. Nephrostomy. Leave the left nephrostomy in place and draining. Prep its exit site into the field if it can be reached. If not, connect a sterile extension line with a three-way stopcock that reaches the field. During the case I instill saline through it to find the ureter, and I may pass an antegrade guidewire through it.Nefrostomía. Dejar la nefrostomía izquierda en su lugar, drenando. Prep and drape. From the nipple line to mid-thigh, and down both flanks to the table. Electrosurgical return pad on a thigh. Warm-air blanket over the chest and arms. Pneumatic compression on the legs.Antisepsia y campos. Desde la línea mamilar hasta la mitad de los muslos, y por ambos flancos hasta la mesa. Placa de electrocauterio en un muslo. Manta de aire caliente sobre tórax y brazos. Compresión neumática intermitente en las piernas.
No urethral catheter. There is no bladder. Urine drains through the conduit and the nephrostomy.Sin sonda uretral. No hay vejiga. La orina drena por el conducto y por la nefrostomía.

If there is only one monitor and tower, put them at the foot of the table, facing the head, so the surgeon, camera holder and assistant all see the screen. Retrograde access through the stoma. The scope enters the conduit to find the old uretero-ileal anastomosis from inside, and to pass a guidewire or catheter up the ureter.Acceso retrógrado por el estoma. El endoscopio entra al conducto para localizar desde dentro la anastomosis ureteroileal previa, y para pasar una guía o un catéter hacia el uréter.
Stent. To place the stent through the conduit, or to check that its lower end sits inside the conduit.Catéter doble J. Para colocar el catéter a través del conducto, o para verificar que su extremo distal quede dentro del conducto.
Identifying the ureter. In dense scar, vessels can look like the ureter. A guidewire or the scope light inside the ureter, from the conduit or antegrade through the nephrostomy tract, shows which structure is the ureter.Identificación del uréter. En tejido cicatricial denso, los vasos pueden parecer el uréter. Una guía o la luz del endoscopio dentro del uréter, desde el conducto o en forma anterógrada por el trayecto de la nefrostomía, indica cuál es el uréter.
If one is available, please have it sterile, with its light source, video processor and monitor (or a connection to the main tower), saline irrigation, and a grasper for the working channel, tested before the patient enters the room. If no flexible cystoscope is available, we will use a catheter instead. If a flexible ureteroscope is available, please have it as well.Si hay uno disponible, favor de tenerlo estéril, con su fuente de luz, procesador de video y monitor (o conexión a la torre principal), irrigación con solución salina y pinza para el canal de trabajo, probado antes de que el paciente entre a sala.
